Hypnotherapy
By choosing to begin Solution Focused Hypnotherapy with Mind Flow Hypnotherapy, I understand and agree to the following: I understand that the number of sessions suggested is a guideline rather than a guarantee, and that progress looks different for everyone. I understand that hypnotherapy works best as a shared process, and that my openness, honesty and engagement play an important role in creating change. I agree to listen to the personalised hypnosis audio regularly, as this helps reinforce the work we do together and supports my progress between sessions. I understand that my therapist works in line with recognised professional and ethical standards, ensuring safe, respectful and responsible practice. I understand that meaningful change is influenced by my intention and readiness to move forward. I agree to gently introduce positive, solution-focused habits or activities between sessions, knowing these small steps help build momentum. I understand that session fees are payable in advance, unless an alternative arrangement has been agreed beforehand. I understand that sessions will begin and end at the agreed time, and that I am required to give at least 24 hours’ notice if I need to cancel or rearrange a session, in line with the Cancellation Policy. If my session takes place online, I confirm that I will be in a private, quiet and confidential space, free from interruption, and visible on camera. I understand that Solution Focused Hypnotherapy does not replace medical care, and that I should seek appropriate medical advice or diagnosis where needed. I understand that everything shared within sessions is treated as confidential, unless there is a legal or safety-related reason to act otherwise. I understand that my therapist attends professional supervision as part of best practice. My work may be discussed anonymously, with no identifying information shared. I understand that my therapist has a professional duty of care, and may need to take appropriate action if there are serious concerns about safety. I understand that brief session notes will be stored securely in line with GDPR regulations and professional insurance requirements. I confirm that the process of Solution Focused Hypnotherapy has been clearly explained to me and that I have had the opportunity to ask questions. I consent to receiving Solution Focused Hypnotherapy with Toby Smith at Mind Flow Hypnotherapy and agree to the above.
Privacy Notice for Clients (GDPR)
How will my data be stored? In May 2018 the Data Protection Act was replaced by the General Data Protection Regulations (GDPR). The changes to the Data Protection Act are aimed at ensuring that your personal, confidential and sometimes sensitive data, is held privately and securely. How long will you hold my information for? In line with NCH guidelines, we keep written records stored securely for 8 years after the last interaction with an adult client and up to the age of 25 for a child under 16 when last seen or 26yrs for 17-18 year old. What if I would like my data to be destroyed before this date? Due to the sensitive nature of what we do, my insurance company does not allow the deletion of data before the minimum time for holding said data. Am I able to see or get a copy of the information held by you? In line with GDPR, yes, within 30 days. Please send your request to toby@mindflowhypnotherapy.co.uk. What are your reasons for collecting this information? We use information held about you in the following ways: Internal record keeping. To provide you with information on the services that you request from us. To carry out our obligations arising from any contracts entered into between you and us. To allow you to participate in interactive features of our service, when you choose to do so. To contact you for your views on our services and to notify you about changes or developments to our service. How do I know that you will store my information securely? Your data will be securely stored as either: Physical documents, kept in a document safe between sessions. Digital documents, saved in a folder accessible only through two-factor authentication. Are our discussions within the hypnotherapy sessions confidential? Yes, unless I need support from my supervisor or I believe that you are about to harm yourself or another. What if I see you outside of a hypnotherapy session? If we do not know each other outside of our therapeutic relationship, I will not acknowledge you outside of sessions unless you initiate contact first. Will you discuss information about me with other health and social care professionals? Only with your written consent.


Cancellation Policy
I understand that life happens, and sometimes plans need to change. To keep things fair for everyone and to respect the time set aside for your session, I ask that any appointments are cancelled or rescheduled with at least 48 hours’ notice. If you give 48 hours’ notice or more, any prepaid session will simply be moved to your next appointment. To cancel or reschedule, please send me a WhatsApp message or call me on 07779 599 031. Sessions cancelled with less than 48 hours’ notice are charged at the full session fee. This helps protect the time reserved for you and allows space for others who may be waiting for support. If a session is missed without any notice, the full session fee will apply. Once I receive your message, I’ll confirm the cancellation and help you rearrange if needed.
